Use this form to submit an online insurance enrollment/waiver request

All full-time students are automatically enrolled in the Student Health Insurance Plan unless the required online insurance enrollment/waiver request is completed by the deadline. The premium is charged only once per year during the first semester of full-time enrollment for the academic year.
